Step 2
Pay Registration Fee
Includes 18 Hours of LEVEL 1 Instruction, workbook, and one drug screen.
Step 3
Schedule Individual Substance Abuse Assessment
Appropriate LEVEL of treatment
(I, II, or III) determined.
Step 4
Complete Initial Drug Screen
Step 5
Complete Pre-Test and all Intake
and Substance Abuse Assessments required by the WV DMV
Step 6
Participate in 18-hour DUI Safety & Treatment Program Courses:
6-hour group session, one day a week, for three consecutive weeks (Accelerated Track)
OR
Step 73-hour group session, one day a week, for six consecutive weeks
You will be asked to provide 2nd random urinalysis during this time and
Complete Post-Test at last group session.
Step 7
Schedule and Complete final assessment & receive certificate of completion, we will mail your packet to the DMV.

DUI Price List:
Second and Additional Drug Screens, as required: $50
*** All participants are required, at a minimum, to have a second/random drug screen.
*** All participants are required to pass an end of class drug screen.
*** WARNING - If you fail a breathalyzer and/or random drug screen you will be advanced to a Level and will be required to pay the additional fees before continuing with treatment.
Substance Abuse Evaluation: $150
This is specific for intervention groups, i.e. (not all inclusive):
High BAC (Blood Alcohol Concentration) at the time of your arrest. DMV deems second offense or greater. If you refused the breathalyzer. Under the age of 21. DUI – Drug related.
Individual Therapy Sessions: $75 per session
Group Therapy Sessions: $60 per session
*** Additional therapy is available for participants, at their discretion. However, depending on client sobriety/failed drug screens, further counseling may be required. This will be discussed on as per case basis with the individual at the time of a failed breathalyzer/drug screen.
Proof of participation letter: $10 (up to 48 hours to process)

*** Becoming a client of Holistic Medical Services and Counseling for ongoing Substance Abuse treatment and Counseling are available options and can be discussed with Holistic providers at any time.
